14Dyletswydd i benodi cydgysylltydd gofal ar gyfer claf perthnasol

(1)Rhaid i'r darparydd gwasanaeth iechyd meddwl perthnasol ar gyfer claf perthnasol benodi unigolyn yn gydgysylltydd gofal ar gyfer y claf i weithredu swyddogaethau ynglŵn â'r claf a roddir i gydgysylltwyr gofal gan ac o dan y Rhan hon.

(2)Mae'r ddyletswydd o dan is-adran (1) i'w chyflawni cyn gynted ag y bydd yn rhesymol ymarferol–

(a)ar ôl i unigolyn ddod yn glaf perthnasol; neu

(b)mewn achos pan fydd unigolyn yn peidio â bod wedi ei benodi'n gydgysylltydd gofal claf perthnasol yn barhaol, ar ôl terfynu'r penodiad hwnnw yn barhaol.

(3)Pan fo'r darparydd gwasanaeth iechyd meddwl perthnasol o'r farn bod cydgysylltydd gofal claf am ba reswm bynnag yn analluog dros dro i weithredu felly, caiff y darparydd benodi unigolyn yn gydgysylltydd gofal dros dro ar gyfer y claf i gyflawni mewn perthynas â'r claf y swyddogaethau y cyfeirir atynt yn is-adran (1).

(4)Mae penodiad dros dro o dan is-adran (3) yn terfynu pan fo'r darparydd gwasanaeth iechyd meddwl perthnasol o'r farn bod yr unigolyn a benodwyd yn flaenorol yn gydgysylltydd gofal wedi adennill y gallu i weithredu felly, ac yn yr achos hwnnw adferir penodiad yr unigolyn hwnnw.

(5)Caniateir i drefniadau gael eu gwneud rhwng dau Fwrdd Iechyd Lleol er mwyn i swyddogaethau'r naill o dan is-adran (1) neu (3) gael eu harfer gan y llall.

(6)Ni fydd unrhyw drefniadau a wneir o dan is-adran (5) yn effeithio ar gyfrifoldeb Bwrdd Iechyd Lleol fel darparydd gwasanaeth iechyd meddwl perthnasol o dan is-adrannau (1) neu (3).

(7)Mae adran 15 yn gwneud darpariaeth ynghylch dynodi'r darparydd gwasanaeth iechyd meddwl perthnasol ar gyfer claf perthnasol.
