Changes to Legislation

Efallai na fydd deddfwriaeth ddiwygiedig sydd ar y safle hwn yn gwbl gyfoes. Ar hyn o bryd mae unrhyw newidiadau neu effeithiau hysbys a wnaed gan ddeddfwriaeth ddilynol wedi'u gwneud i destun y ddeddfwriaeth yr ydych yn edrych arni gan y tîm golygyddol. (1)Dyma yw gwasanaethau cymorth iechyd meddwl sylfaenol lleol–

(a)cynnal asesiadau iechyd meddwl sylfaenol yn unol â darpariaethau canlynol y Rhan hon;

(b)darparu ar gyfer unigolyn, yn dilyn asesiad iechyd meddwl sylfaenol, driniaeth iechyd meddwl sylfaenol leol a ddynodir gan yr asesiad yn driniaeth a allai wella iechyd meddwl yr unigolyn neu atal dirywiad ynddo;

(c)gwneud atgyfeiriadau fel a ddisgrifir yn adran 10, yn dilyn asesiad iechyd meddwl sylfaenol, ynghylch gwasanaethau eraill y gallai eu darparu wella iechyd meddwl yr unigolyn a aseswyd neu atal dirywiad ynddo;

(d)darparu gwybodaeth, cyngor a chymorth arall i ddarparwyr iechyd sylfaenol i fodloni gofynion rhesymol y darparwyr am y cyfryw wybodaeth, cyngor a chymorth arall at ddibenion gwella'r gwasanaethau mewn perthynas ag iechyd meddwl y maent yn eu darparu neu'n eu trefnu;

(e)darparu gwybodaeth a chyngor ar gyfer cleifion a'u gofalwyr ynghylch y gwasanaethau sydd ar gael iddynt i fodloni eu gofynion rhesymol am y cyfryw wybodaeth a chyngor.

(2)Yn is-adran (1)(e)–

  • ystyr “cleifion” (“patients”) yw unigolion y mae ganddynt, neu y gall fod ganddynt, anhwylder meddwl;

  • ystyr “gofalwyr” (“carers”) yw aelodau o deuluoedd cleifion, a ffrindiau cleifion, sy'n ymwneud â'u gofal ac yn achos claf sy'n blentyn mae'n cynnwys rhiant maeth awdurdod lleol y plentyn (o fewn ystyr [F1adran 105(1)] o Ddeddf Plant 1989);

  • ystyr “gwasanaethau” (“services”) yw–

    (a)

    gwasanaethau iechyd meddwl eilaidd;

    (b)

    gwasanaethau gofal cymunedol (nad ydynt yn wasanaethau iechyd meddwl eilaidd);

    (c)

    gwasanaethau a ddarperir o dan [F2adrannau 37 i 39 a Rhan 6 o Ddeddf Gwasanaethau Cymdeithasol a Llesiant (Cymru) 2014] (nad ydynt yn wasanaethau iechyd meddwl eilaidd);

    (d)

    gwasanaethau tai neu wasanaethau llesiant; a

    (e)

    addysg neu hyfforddiant a all fod yn llesol i iechyd meddwl claf.
