Diwygio o Rheoliadau Gwasanaethau Fferyllol 1992

2.—(1) Diwygir Rheoliadau Gwasanaethau Fferyllol 1992 fel a ganlyn.

Ym mharagraff (1) o reoliad 2 (dehongli)—

(3) Hepgorwch baragraffau (1A)(5) ac (1C)(6) o reoliad 2.

(4) Yn rheoliad 18(1) (safonau cyffuriau ac offer a thalu amdanynt), yn is-baragraff (a), ar y diwedd, mewnosodwch “and, in the case of a restricted availability appliance, the categories of persons for whom or purposes for which the appliance is approved”.

(5) Yn rheoliad 19 (darparu gwasanaethau fferyllol ar gyfer triniaeth ar unwaith neu weinyddu'n bersonol)(7), ar y diwedd ychwanegwch “ but shall, in either case, provide a restricted availability appliance only if it is for a person or a purpose specified in the Drug Tariff.”.

(6) Yn Rhan II of Atodlen 2 (telerau gwasanaeth ar gyfer fferyllwyr)—

(a)ym mharagraff (3)(1)(8))—

(i)yn is-baragraff (a), ar ôl “or appliances,” mewnosodwch “not being restricted availability appliances,”;

(ii)ar ôl is-baragraff (b), mewnosodwch—

“(bb)an order for a restricted availability appliance, signed by and endorsed on its face with the reference “SLS” by a doctor; or”;

(iii)yn lle is-baragraff (d), rhowch—

“(d)an order for drugs or medicines, or listed appliances, not being restricted availability appliances, signed by a nurse prescriber”; a

(iv)ar ôl is-baragraff (d), mewnosodwch—

“or

(e)an order for a restricted availability appliance, signed by and endorsed on its face with the reference “SLS” by a nurse prescriber,”;

(b)ym mharagraff (11)(9)—

(i)yn is-baragraff (b), dilewch “and”;

(ii)yn is-baragraff (c), ar y diwedd, mewnosodwch “; and”; a

(iii)ar ôl is-baragraff (c), ychwanegwch—

“(d)he shall provide the for patient a restricted availability appliance only if the patient is a person, or it is for a purpose, specified in the Drug Tariff.”;

(c)ym mharagraff 11A(10)—

(i)yn lle is-baragraff (2), rhowch—

“(2) Where a patient presents an order on a prescription form for drugs or medicines or listed appliances signed by a nurse prescriber, or an order for a restricted availability appliance signed by and endorsed on its face with the reference “SLS” by a nurse prescriber, to a doctor who is authorised or required by regulation 20 to provide drugs or appliances to that patient, the doctor may provide to the patient such of the drugs, medicines or appliances so ordered as he supplies in the normal course of his practice.”; a

(ii)yn is-baragraff (3), hepgorwch y gair “listed” y tro cyntaf y mae'n digwydd;
