Gofynion y mae rhaid i ymarferydd cymwysedig sydd wedi ei gynnwys mewn rhestr atodol gydymffurfio â hwy

16.—(1) Wrth ddod yn ymwybodol o newid i’r wybodaeth a ddarparwyd gan yr ymarferydd cymwysedig yn unol â pharagraffau 5 i 7 o Atodlen 3 wrth wneud cais i gael ei gynnwys yn rhestr atodol y Bwrdd Iechyd Lleol, rhaid i’r ymarferydd cymwysedig hysbysu’r Bwrdd Iechyd Lleol yn ysgrifenedig o fewn 7 niwrnod.

(2) Pan fo is-baragraff (1) yn gymwys, rhaid i’r ymarferydd cymwysedig ddarparu’r holl awdurdod angenrheidiol er mwyn galluogi i gais gael ei wneud gan y Bwrdd Iechyd Lleol i unrhyw gyflogwr (neu gyn-gyflogwr), unrhyw gorff trwyddedu, unrhyw gorff rheoleiddio neu unrhyw gorff arall yn y Deyrnas Unedig neu yn rhywle arall, am wybodaeth sy’n ymwneud â’r hysbysiad a roddir gan yr ymarferydd cymwysedig o dan is-baragraff (1).

(3) Rhaid i ymarferydd cymwysedig sydd wedi ei gynnwys yn rhestr atodol Bwrdd Iechyd Lleol gyflenwi i’r Bwrdd Iechyd Lleol hwnnw dystysgrif cofnod troseddol manwl o dan adran 113B o Ddeddf yr Heddlu 1997(1) mewn perthynas â’r ymarferydd cymwysedig os yw’r Bwrdd Iechyd Lleol ar unrhyw adeg, am achos rhesymol, yn rhoi hysbysiad i’r ymarferydd cymwysedig i ddarparu tystysgrif o’r fath.
