Gwneud cais am y premiwm cigydda

4.—(1) Rhaid i gais mewn perthynas â llo premiwm, anifail premiwm mewn oed ac anifail premiwm hŷn gael ei gyflwyno i'r Bwrdd a rhaid i bob cais fod ar unrhyw ffurf a chynnwys unrhyw fanylion, yn ychwanegol at yr wybodaeth sy'n ofynnol o dan reolau'r Gymuned, ac unrhyw ddogfennau, y bydd y Bwrdd yn gofyn yn rhesymol amdanynt.

(2) Gyda chais mewn perthynas â llo premiwm sy'n bum mis oed o leiaf adeg ei gigydda, rhaid anfon y cofnod ysgrifenedig ynghylch ei gigydda y cyfeirir ato ym mharagraff 5 o'r Atodlen.

(3) Caiff cais a gyflwynir yn unol â pharagraff (1) fod yn gais mewn perthynas ag unrhyw nifer o loi premiwm, anifeiliaid premiwm mewn oed neu anifeiliaid premiwm hŷn, yn ôl fel y digwydd.
