Adroddiadau blynyddol

16.—(1) Rhaid i'r awdurdod penodi baratoi a chymeradwyo, mewn perthynas â phob fforwm y mae'n awdurdod penodi arno, adroddiad blynyddol cyn gynted ag y bo'n rhesymol ymarferol ar ôl 31 Mawrth ym mhob blwyddyn.

(2) Rhaid i adroddiad blynyddol a baratoir o dan baragraff (1) gynnwys—

(a)gwybodaeth ynglyn â pherfformiad y fforwm o'i swyddogaethau yn ystod y cyfnod o 12 mis yn diweddu ar y dyddiad y cyfeirir ato ym mharagraff (1); a

(b)rhaglen waith y cytunwyd arni rhwng y fforwm a'r awdurdod penodi y mae'r fforwm yn bwriadu ymgymryd â hi yn ystod y 12 mis sy'n dechrau yn union ar ôl y dyddiad hwnnw.

(3) Rhaid sicrhau bod copi o'r adroddiad blynyddol, wedi iddo gael ei gymeradwyo, ar gael ar gyfer archwiliad gan aelodau o'r cyhoedd yn swyddfeydd yr awdurdod penodi yn ystod oriau swyddfa arferol yn rhad ac am ddim.

(4) Rhaid i awdurdod penodi, ar gais unrhyw aelod o'r cyhoedd, ddarparu copi i'r person hwnnw o unrhyw adroddiad blynyddol sydd wedi'i baratoi a'i gymeradwyo o dan y Rheoliad hwn, neu unrhyw ran ohono, pan delir cost y postio ac unrhyw swm pellach nad yw'n fwy na 10 ceiniog y tudalen y bydd yr awdurdod penodi yn gofyn amdano.
