Hysbysiad o gynnig i faethu plentyn yn breifat

3.—(1) Rhaid i berson sy'n cynnig maethu plentyn yn breifat hysbysu'r awdurdod lleol priodol o'r cynnig—

(a)o leiaf chwe wythnos cyn bod y trefniant maethu preifat i gychwyn; neu

(b)os yw'r trefniant maethu preifat i gychwyn o fewn chwe wythnos, ar unwaith.

(2) Rhaid i unrhyw berson sy'n ymwneud a threfnu (p'un ai'n uniongyrchol ai peidio) i blentyn gael ei faethu'n breifat hysbysu'r awdurdod lleol priodol o'r trefniant cyn gynted â phosibl ar ôl i'r trefniant gael ei wneud.

(3) Rhaid i riant plentyn, a pherson nad yw'n rhiant ond bod ganddo gyfrifoldeb rhiant am y plentyn, nad yw'n ymwneud â threfnu (p'un ai'n uniongyrchol ai peidio) ar gyfer maethu'r plentyn yn breifat, ond sy'n gwybod bod cynnig i faethu'r plentyn yn breifat, hysbysu'r awdurdod lleol priodol o'r cynnig cyn gynted â phosibl ar ôl dod yn ymwybodol o'r trefniant.

(4) Rhaid i hysbysiad a roddir o dan baragraffau (1) i (3) gynnwys cymaint o'r wybodaeth honno a bennir yn Atodlen 1 y mae'r person sy'n rhoi'r hysbysiad yn gallu ei darparu.
