Hysbysiad o newid mewn amgylchiadau

9.—(1) Rhaid i ofalydd maeth preifat hysbysu'r awdurdod lleol priodol—

(a)o unrhyw newid yn ei gyfeiriad;

(b)o unrhyw berson sy'n peidio â bod yn rhan o'i aelwyd neu'n gyflogedig ar ei aelwyd;

(c)o unrhyw dramgwydd pellach y collfarnwyd y gofalydd maeth preifat ohono neu berson sy'n rhan o'i aelwyd neu a gyflogir ar ei aelwyd;

(ch)o unrhyw ddatgymhwysiad pellach a osodwyd arno neu ar berson sy'n rhan o'i aelwyd neu a gyflogir ar ei aelwyd o dan adran 68 o'r Ddeddf; ac

(d)os oes unrhyw berson yn dechrau bod yn rhan o aelwyd y gofalydd maeth preifat neu'n gyflogedig ar ei aelwyd, ac unrhyw dramgwydd y collfarnwyd y person hwnnw ohono, ac unrhyw ddatgymhwysiad neu waharddiad a osodwyd arno o dan adran 68 neu 69 o'r Ddeddf neu o dan unrhyw ddeddfiad blaenorol o unrhyw un o'r ddwy adran honno.

(2) Rhaid i hysbysiad o dan baragraff (1) gael ei roi—

(a)ymlaen llaw pan fo hynny'n ymarferol;

(b)ym mhob achos arall, dim mwy na 48 awr ar ôl y newid mewn amgylchiadau.

(3) Os bydd cyfeiriad newydd y gofalydd maeth preifat yn ardal awdurdod lleol arall, neu awdurdod lleol yn Lloegr, yr Alban neu Ogledd Iwerddon, rhaid i'r awdurdod y cyflwynir yr hysbysiad iddo o dan y rheoliad hwn anfon at yr awdurdod dros yr ardal—

(a)enw a chyfeiriad newydd y gofalydd maeth preifat;

(b)enw'r plentyn a faethir yn breifat; ac

(c)enw a chyfeiriad rhieni'r plentyn neu unrhyw berson arall sydd â chyfrifoldeb rhiant am y plentyn.

(4) Rhaid i riant plentyn a faethir yn breifat, ac unrhyw berson arall sydd â chyfrifoldeb rhiant am y plentyn, sy'n gwybod bod y plentyn yn cael ei faethu yn breifat, hysbysu'r awdurdod lleol priodol o unrhyw newid yng nghyfeiriad personol y person hwnnw.
