2.  Yn achos person sy'n rhoi hysbysiad o dan reoliad 3(1) neu 5(1) mae'r wybodaeth y cyfeirir ati yn rheoliad 3(4) a 5(2) hefyd yn cynnwys—

(a)unrhyw dramgwydd y collfarnwyd ef ohono;

(b)unrhyw ddatgymhwysiad neu waharddiad a osodwyd arno o dan adran 68 neu 69 o'r Ddeddf neu o dan unrhyw ddeddfiad blaenorol o unrhyw un o'r adrannau hynny;

(c)unrhyw gollfarn, datgymhwysiad neu waharddiad o'r fath a osodwyd ar unrhyw berson arall sy'n byw ar yr un aelwyd neu a gyflogir ar yr un aelwyd.

(ch)unrhyw orchymyn o fath a bennir mewn rheoliadau o dan adran 68 o'r Ddeddf a wnaed ar unrhyw adeg yn ei gylch;

(d)unrhyw orchymyn o fath a bennir mewn rheoliadau o dan adran 68 o'r Ddeddf a wnaed ar unrhyw adeg o ran plentyn a fu yn ei ofal; ac

(dd)unrhyw hawl neu bŵer ynglŷn â phlentyn sydd neu a fu o dan ei ofal a gafodd eu breinio ar unrhyw adeg mewn awdurdod a bennwyd mewn rheoliadau o dan adran 68 o'r Ddeddf o dan ddeddfiad a bennir yn y rheoliadau hynny.
