Rheoliad 4

ATODLEN 2LLES PLANT SYDD I'W MAETHU YN BREIFAT

Y materion y cyfeiriwyd atynt yn rheoliad 4(1)(d) yw—

(a)bod hyd arfaethedig y trefniant wedi cael ei ddeall a'i gytuno rhwng—

(i)rhiant y plentyn neu unrhyw berson arall â chyfrifoldeb rhiant am y plentyn; a

(ii)y gofalydd maeth preifat arfaethedig;

(b)dymuniadau a theimladau'r plentyn am y trefniant arfaethedig (a'u hystyried yng ngoleuni ei oedran a'i ddeall);

(c)addasrwydd y llety arfaethedig;

(ch)gallu'r gofalydd maeth preifat arfaethedig i edrych ar ôl y plentyn;

(d)addasrwydd aelodau eraill o aelwyd y gofalydd maeth preifat arfaethedig;

(dd)bod y trefniadau a gytunir ar gyfer cyswllt rhwng y plentyn a'i rieni, personau eraill sydd â chyfrifoldeb rhiant amdano, a phersonau eraill sy'n arwyddocaol i'r plentyn, wedi cael eu cytuno a'u deall ac y bydd y trefniadau hynny'n foddhaol ar gyfer y plentyn;

(e)bod y rhieni neu berson arall sydd â chyfrifoldeb rhiant am y plentyn wedi cytuno ar drefniadau ariannol â'r gofalydd maeth preifat arfaethedig ar gyfer gofalu am y plentyn a'i gynnal;

(f)bod ystyriaeth wedi'i rhoi a'r camau angenrheidiol wedi'u cymryd i wneud trefniadau ar gyfer gofal meddygol, deintyddol ac optegol a thriniaeth feddygol, ddeintyddol ac optegol y plentyn;

(ff)bod ystyriaeth wedi'i rhoi a'r camau angenrheidiol wedi'u cymryd i wneud trefniadau ar gyfer addysg y plentyn;

(g)sut y gwneir penderfyniadau ynghylch gofal y plentyn; ac

(ng)a yw'r gofalydd maeth preifat arfaethedig, rhieni'r plentyn, unrhyw berson arall sydd â chyfrifoldeb rhiant am y plentyn, neu unrhyw berson arall sy'n ymwneud â'r plentyn yn cael y cyngor hwnnw y mae'n ymddangos i'r awdurdod bod ei angen.
