Rheoliadau 7 ac 8

ATODLEN 3LLES PLANT A FAETHIR YN BREIFAT

3.  Y materion y cyfeiriwyd atynt yn rheoliadau 7(1)(d) ac 8(4) yw—

(a)bod diben a hyd arfaethedig y trefniant maethu yn cael ei ddeall a'i gytuno rhwng—

(i)rhieni'r plentyn neu unrhyw berson arall â chyfrifoldeb rhiant am y plentyn; a

(ii)y gofalydd maeth preifat;

(b)dymuniadau a theimladau'r plentyn am y trefniant (a'u hystyried yng ngoleuni ei oedran a'i ddeall);

(c)bod datblygiad corfforol, deallusol, emosiynol a chymdeithasol y plentyn a datblygiad ei ymddygiad yn briodol a boddhaol;

(ch)bod anghenion y plentyn sy'n codi o'i gred grefyddol, ei darddiad hiliol, a'i gefndir diwylliannol ac ieithyddol yn cael eu diwallu;

(d)bod y trefniadau ariannol ar gyfer gofalu am y plentyn a'i gynnal yn gweithio;

(dd)gallu'r gofalydd maeth preifat i edrych ar ôl y plentyn;

(e)addasrwydd y llety;

(f)bod y trefniadau ar gyfer gofal meddygol, deintyddol ac optegol a thriniaeth feddygol, ddeintyddol ac optegol, wrth law, ac yn benodol, fod y plentyn wedi'i roi ar restr person sy'n darparu gwasanaethau meddygol sylfaenol yn unol â Rhan 1 o Ddeddf y Gwasanaeth Iechyd Gwladol 1977;

(ff)y trefniadau ar gyfer addysg y plentyn;

(g)safon y gofal a roddir i'r plentyn;

(ng)addasrwydd aelodau aelwyd y gofalydd maeth preifat;

(h)a yw'r gofalydd maeth preifat, rhieni'r plentyn, unrhyw berson arall sydd â chyfrifoldeb rhiant am y plentyn, neu unrhyw berson arall sy'n ymwneud â'r plentyn yn cael y cyngor hwnnw y mae'n ymddangos i'r awdurdod bod ei angen;

(i)sut y gwneir penderfyniadau am ofal y plentyn; ac

(l)a yw'r cyswllt rhwng y plentyn a'i rieni, neu unrhyw berson arall y trefnwyd cyswllt ag ef, yn foddhaol ar gyfer y plentyn.
